I'll probably just put some reinforcement in there to see what happens rather than make a new top. As I said in my original post, I posted this here rather than the repair section because I was more interested in understanding why it happened and how to prevent it on future guitars.
@Doug, I suppose that's a possibility and something like that must have happened, but we use a LOT of glue in our shop. We buy it by the gallon and it usually doesn't last very long. But who knows, maybe I grabbed an old bottle that was just lying around that day.

Ryan: Some folks advocate for leaving out the popsicle brace. I don't and it is because of this type of disaster. I do shorten the brace but make sure it goes at least 1/2" or so past the O.D. of the sound hole on either side. For this to happen the glue line on the UTB had to fail so if you are running into creep from over heating ,shift to HHG or be more careful where you put the guitar. Hope you get it resolved.
